-- We require that a Referral Information Form (blue button below or above on this page) be filled out in order to initiate a request for neuropsychological testing.

-- We are conducting neuropsychological testing/evaluations in-office / in-person only at this time. 

-- In order to accept a request for a neuropsychological evaluation, we kindly ask that you complete a Referral Information Form (blue Request an Evaluation button). If you are a health practitioner referring a client, we request that you please ask your client to also fill out a Referral Information Form. 

-- Our focus is neuropsychological testing  / evaluations , although we do provide a list of potential therapists  in our reports based on the issues identified. ​
