Please note:
1. We are scheduling about 1-2 months out.
2. If you would like to self-pay for an evaluation, and would like to know the cost, please email us at dreneidasilva@gmail.com.
3. In order to accept a request for a neuropsychological evaluation, we need that you complete a Referral Information Form (blue Request an Evaluation button). If you are a health practitioner referring a client, we request that you please ask your client to also fill out a Referral Information Form, as this is needed for us to accept a request for an evaluation and initiate the process.
4. Our focus is neuropsychological testing / evaluations , although we do provide a list of potential therapists in our reports based on the issues identified. ​
